- Work closely with case managers and prevention staff to reduce barriers and help patients successfully connect with needed services
- Provide culturally responsive, trauma-informed, and affirming care
Collaboration & Program Development
- Collaborate with case management, prevention staff, behavioral health, pharmacy, phlebotomy, front-desk staff, and other community partners
- Work closely with the Medical Director and clinical leadership to provide coordinated patient care
- Participate in quality improvement efforts and the continued development of the low-barrier clinic model
- Identify opportunities to improve workflows, referral processes, rapid-treatment pathways, and the overall patient experience
- Take initiative in recommending solutions and improvements as services continue to grow
