Full-time position in Nashville, TN
Compensation: $33,000 – $42,000 per year
Job Summary:
Nashville CARES is seeking a passionate Patient Navigator to join our HealthyU initiative! You will work on-site at the main campus and serve as a bridge between clients and their primary case manager. You will primarily work with in-person clients who walk in, clinic clients who reach out for assistance, and new clients who are having an intake with their case manager. You will also provide referral and resource information to community members to facilitate access to CARES or other community services.

What You’ll Do:
- Provide compassionate and respectful care, along with comprehensive peer services, to help clients address the complex challenges associated with living with HIV.
- Adhere to strict confidentiality procedures, the release of information, and data security.
- Assist in increasing knowledge of HIV and PrEP.
- Meet regularly with clients in the office and attend provider appointments with them, if requested, especially for their initial appointment with a provider.
- Communicate information regarding peer groups, nutrition, transportation, and practical support services for clients with an identified need or desire for these services.
- Assist clients with connections to medical, social, community, financial, and other services as identified by their case manager.
- Advocate for clients within the agency and in external settings, such as with medical providers and other agencies they may be engaged with in the community.
- Complete all documentation in ClientTrack in a timely and accurate manner, according to agency standards, to ensure clients do not encounter barriers to service.
- Practice appropriate boundaries with clients and in your work to ensure excellent service while preserving self-care.
- Adhere to project, agency, department, and team policies and procedures.
- Participate in projects, agency, department, and team meetings, events, and strategic initiatives.
- Perform other duties as assigned by the supervisor and agreed upon by the HealthyU Navigator.
What You’ll Need:
- High school diploma or GED
- 3 years of experience working closely with HIV+ clients or in similar community-based programs
- Lived experience with HIV is strongly preferred, as this role involves direct peer support and advocacy for individuals living with HIV.
- Driver’s license, clean DMV record, and automobile liability insurance required
- Computer proficiency. Previous experience with ClientTrak and Microsoft Suite is a plus.
- Ability to maintain a flexible schedule, with excellent communication and interpersonal skills.
