Full-time position in Nashville, TN
Compensation: $ 95,000 to $110,000 a year (DOE)
Job Summary:
The Clinic RN – HIV Engagement and Retention in Care Specialist will play a critical role in improving health outcomes for people living with HIV by facilitating entry into care, supporting long-term retention, and identifying and addressing barriers to care.
This position combines clinical nursing duties with patient navigation, case management, care coordination, and population health strategies focused on HIV prevention and the HIV continuum of care.

What You’ll Do:
- Conduct outreach and follow-up for patients newly diagnosed with HIV, those out of care, or those at risk of falling out of care.
- Collaborate with care teams to ensure timely ART initiation and linkage to medical, psychosocial, and supportive services.
- Monitor patient panels for missed appointments, gaps in monitoring, or treatment adherence concerns.
- Participate in multidisciplinary case conferences to address complex care needs and optimize patient retention.
- Educate patients on HIV disease, treatment adherence, U=U, prevention of transmission, and other relevant health topics.
- Serve as a liaison between patients, providers, and community resources to facilitate referrals and close care gaps.
- Utilize data reports and care management tools to proactively identify and address patient needs.
What You’ll Need:
- Licensed Registered Nurse (RN) in Tennessee; BSN preferred.
- Minimum 10 years of clinical experience in HIV or public health.
- Strong understanding of the HIV care continuum and social determinants affecting retention in care.
- Familiarity with Ryan White programs.
- Excellent communication and interpersonal skills; ability to work independently and as part of a diverse team.
